Promenade Healthcare requires written parental consent for the treatment of any minor unless the minor receives Medicaid. If your child is under the age of 18 and will be seen without you present, you will need to sign a Parental Consent form and it will need to be presented at each visit before your child will be seen by the nurse practitioner. Parental Consent forms are located in our Forms section on our website and all information completed on behalf of the minor patient will be verified prior to services rendered.
Although, we are extremely diligent in the verification of information submitted, we must place a caveat of awareness that Promenade Healthcare is not responsible for the falsification of any information submitted on consent forms presented to our office.
If you require a refill on a current medication, please call your pharmacy first to check if you have any refills remaining on your prescription. If not, ask your pharmacist to contact our office and leave a message to request a prescription refill. Although we try our best to have prescription renewals processed, it could take up to 48 hours for your prescription to be refilled. Therefore, please do not wait until you are out of your medication.
Depending on the length of time the nurse practitioner has seen you, you may be required to schedule an office visit and have labs drawn for new scripts written on certain medications. If you have not been seen within a specified period of time, you will receive notification that you are due for an office visit. If you are not able to make your visit we may deny the refill altogether; no exceptions.
We do not prescribe narcotics. Please do not ask us to deviate from this policy.
Normal Results
It is not the policy of Promenade Healthcare to call all patients with normal test results. If you have any questions about your test results, please feel free to call our office. Your call will be returned by a staff member and they will be happy to discuss your results with you. Please allow up to 1week for receipt and processing of test results, depending on the type of test you have administered. If you have not received test results within the time indicated, please do not assume “no news is good news.” Please call our office.
Abnormal Results
You will be notified by telephone of any abnormal test results. We do not leave messages on voicemail with any type of result information. We do, however, leave a message asking you to return our call. On occasion, we may request that you come in to our office or schedule an appointment to discuss complicated or sensitive test results. We reserve the right to require an office visit before releasing test results to you.
If you are 18 years of age or older, it is a HIPAA violation (Health Insurance Portability and Accountability Act) for us to release your results to anyone else without your specific written consent. Forms for consenting to the release of information can be obtained in our Forms section of our website.
In the event that we cannot reach you by telephone after at least three attempts, or you do not respond to messages left on voicemail, we will issue a letter and mail it to your address of record via Certified, Return Receipt Mail asking that you contact us. If you do not respond, Promenade Healthcare will no longer be obligated to attempt to contact you. Any harm that comes to you as a result of your failure to contact our office is not the responsibility of Promenade Healthcare, PLLC. It is also your responsibility to make sure we have accurate phone numbers and addresses in the event that we should need to contact you. Please verify this information each time you visit our office.
We work very hard to accommodate appointment schedules for our patients. Appointments can be made by calling our office during regular business hours as indicated on our website.
For all appointments, a front desk associate will update and verify information needed in order for us to see you and provide for your health care needs. It is important to have your insurance card available when calling, as this information is needed when scheduling your appointment. This allows us to verify benefits before your visit.
New patients need to arrive 30 minutes ahead of their scheduled appointment times for the completion of necessary paperwork. New patient and annual patient forms are located under the Forms section of our website to give patients an opportunity to have required forms completed before their visit.
Established patients need to arrive 15 minutes prior to their scheduled appointment to allow adequate time for completion of required forms and update any demographic information.
For all patients who are more than thirty minutes late for their appointment and have not called to let us know, you will either be asked to reschedule your appointment or you will be worked in at the end of the appointment schedule so as not to delay other patients scheduled appointments.
We understand at times that daily schedules are hard to manage, and if you feel you will not be able to make your appointment please be courteous and call our office to reschedule within 24 hours to allow us to schedule another patient in your place. Patients who routinely miss appointments may be discharged from the practice.
As a service to our patients, Promenade Healthcare bills insurance companies for office visits and affiliated charges. If we are unable to verify your insurance, you will be expected to pay by cash or credit card for services performed that day. You will be provided an itemized receipt so you can in turn file a claim for reimbursement from your insurance provider.
All co-payments are requested at check-in and cannot be waived due to our contractual agreements with insurance providers. Patients are responsible for paying co-payments, deductibles, co-insurance and charges denied by your insurance company at each visit.
It is the responsibility of each patient to know their benefits and what services are included in their plans and which ones are not.

Our patients are our number one priority, and that includes keeping you happy, comfortable and well informed of your personal health issues. We not only want to continue our relationships with each one of our patients, we want to make those relationships even better.
We welcome your feedback whether it is of a complimentary nature or if it is a complaint or concern you may have experienced while visiting our office. If you feel that an area of our practice needs attention, please feel free to address any issues with Dr. Johnson-Aubert, FNP-C or another staff member. We continuously strive to meet our patients’ needs as efficiently and effectively as possible. We appreciate your efforts in keeping any complaints or comments constructive.
In accordance with state and federal laws, Promenade Healthcare will not release any medical or personal information to anyone, including but not limited to your spouse, without your specific written consent. This is applicable to any patient over the age of 18 years old. For release of information on patients under the age of 18, the parent or legal guardian will be required to sign a release form. Please keep in mind, our policies regarding these matters are governed by law, and are therefore not flexible; no exceptions will be made.
Patient medical records are the property of Promenade Healthcare. If you would like a copy of your medical records to provide to another physician, you will be required to sign a Medical Records Release Form. This form is located in our Forms section of our website. We release records directly to you to pick up in person, via US mail, or records can be faxed to facility of choice. Please allow up to 7-10 business days for the processing of your records.
Promenade Healthcare understands the need for patients to have certain forms completed. All forms need to be fully completed and signed by the patient before they are given to our Front Office Receptionist when checking in for your appointment. Any form not completed and signed will be given back for completion. Please allow 7 to 10 business days for the forms to be completed.
After completion, someone from our staff will contact the patient to let them know the forms are ready for pickup or if preferred, they can be mailed directly to the patient. We take no responsibility for any forms lost in the mail. Please note that it is the patient’s responsibility to provide the completed forms to intended recipient. Because of the important nature and medical information contained on these forms, we no longer fax these forms on behalf of patients, so please do not ask us to deviate from this policy.
Promenade Healthcare does charge a fee for completion of these forms, and payment is required when turning them in to the office. No exceptions will be made.
Please do not hand these forms to the nurse practitioner, as they see numerous patients a day and forms can be misplaced.
